2. Please bring the following with you during your consultation:

• a. Ballpen (NO SHARING of pens with other patients)

• b. Mask (NO MASK, NO ENTRY)

• c. Alcohol

• d. Laboratory Results

3. On the day of consult, please text or call FIRST to avoid congestion inside the clinic.

4. May proceed inside the building once the secretary confirms that it will be your turn for consult.

5. You will be given a form to be filled up prior to entering the clinic. Be honest with your answers.

6. You will be asked to go through disinfection process prior to going inside the clinic. Foot bath, hand hygiene and temperature check are mandatory.

7. Consultation time is limited to 5-10 minutes only. Hence it is really required for you to contact first the secretary prior to going to the clinic so that laboratory tests results are with you already.

8. BE HONEST AT ALL TIMES.

9. We will be cleaning the area every after consultation hence the need for you to wait for your turn to be called.

10. Please observe proper social distancing at all times.

11. Please prepare exact amount of money for your consultation fee. The secretary will inform you in advance how much you should prepare.

12. After consultation, you are advised to go home immediately and take a full bath.

We live in challenging times right now and you may not be able to see your doctor personally due to the present pandemic situation. Please be informed that our online medical consultation is now available for your convenience. Please be informed also that said medical consultation will incur fees based on standard consultation fees of the physician for a maximum of 30 minutes.

TERMS & CONDITIONS FOR THE ONLINE DOCTOR CONSULTATION
Our online consultation service provides professional medical advice which may include drug prescription with instructions for proper dosage, laboratory requests, appropriate first aid and general healthcare information.
The medical advice given by the doctor during the online consultation are for non-emergency cases and will be based on the information and/or images shared by the patient. It is not a substitute for cases that need actual physical assessment or tests.
Note that every online consultation is only for one patient at a time.
The patient (or guardian for minors / seniors) shall assume full responsibility for the information they provide to the doctor during the online consultation. If you believe that you (or your child/ senior under your care) need immediate medical attention, please go to the nearest medical emergency facility.

PRIVACY POLICY
The patient agrees to provide pertinent personal information that will be part of the patient record. The medical advice is applicable only to the patient present during the online consultation and should not be shared with other individuals. The patient and the doctor are not allowed to record or capture screenshots of any part of the consultation without consent of both parties. Any recording or screenshots of the consultation, whether in whole or in part, which is unauthorized or without the consent of both parties, shall be illegal.

CONSULTATION FEE
• Online doctor consultation fee is *P500.00 for a maximum of 30 minutes. • Image of the proof of payment or bank transfers should be sent via messenger on or before the day of the appointment. • Proof of payment should be confirmed first by the doctor’s secretary before the patient can proceed with the online doctor consultation. • A consultation fee of *P500.00 will apply for follow up online consultation. • An image of the official receipt will be sent to the patient via messenger after the consultation.

REFUNDS AND CANCELLATION POLICY
In case the patient is not able to show up for the online appointment, the slot will be given to the next available patient in line. Patient will be allowed to reschedule at the next availability of the doctor.
In case the patient has been accidentally overcharged, he/ she may send a notification via FB messenger and request for a refund.
In case an advanced payment is made by the patient and he/ she decides to cancel the appointment, he / she may request for a refund. Cancellation of appointments for the 3rd time will forfeit the advanced payments made
